Skills : Basic fundamental knowledge in Pharmacology
Job Description:
The Clinical Pharmacist is responsible for the following:
o Checking the indents for medication errors.
o Checking the home medications and registering them in a prescribed format
o Updating the drug formulary
o Preparing the Chemo admixtures whenever required
o Doing ward rounds and discharge summary check
o Monitoring ADR and documenting the same
o Responding to drug related queries of all health care professionals
o Preparing and submitting monthly reports to Clinical Pharmacologist
